Clarification on the List of Documents Submitted by Applicants

1. Applicants for professional pre-higher education on the basis of basic secondary education (BSE), complete general secondary education (CGSE) or a qualified worker (QW) qualification register applications:

• only in electronic form (through the electronic account in the USEDE on the website at https://vstup.edbo.gov.ua/), except for the cases specified in this paragraph;

submit the first application in paper form with the simultaneous creation of an electronic account at the Admissions Committee of the educational institution, and register other applications only in electronic form:

• if there are discrepancies between the applicant's data in the USEDE (surname, first name, patronymic (if any), date of birth, sex, citizenship, etc.) and the relevant document on the previously obtained educational (educational and qualification) level or degree of professional pre-higher or higher education and in the certificate of the National Multi-subject Test or External Independent Testing (if submitted);

• if a foreign document on education is submitted;

• if a document on complete general secondary education is submitted that was issued before the introduction of photo-polymer technologies for producing such documents;

submit applications in paper form only:

• if documents are submitted by foreigners and stateless persons, except for persons with a permanent residence permit in Ukraine;

• if it is impossible to register an electronic account or submit an application in electronic form for other reasons, as confirmed by a certificate from the Admissions Committee of the professional pre-higher education institution.

2. The applicant attaches the following to an application submitted in paper form:

- a copy of the identity document (one of the documents) provided for by the Law of Ukraine “On the Unified State Demographic Register and Documents Confirming Citizenship, Proving Identity or Special Status”;

- a copy of the birth certificate;

- a copy of the taxpayer identification number if it is not shown on the ID card or the birth certificate;

- a copy of the extract (certificate) of the place of residence (registration);

- a copy of the military registration document (military ID cards or temporary certificates for those liable for military service, and certificates of registration at the conscription office for conscripts), except in cases provided for by legislation;

- a copy of the document (of the state-issued model) on the previously obtained educational (educational and qualification) level, degree of professional pre-higher or higher education, on the basis of which the applicant is admitted, if the information on it is not stored in the Unified State Electronic Database on Education, and a copy of its supplement;

- a copy of the documents on special social status for applicants in preferential categories;

- eight colour photographs measuring 3 x 4 cm;

- a motivation letter submitted in electronic form (in electronic form or in paper form for persons who submit an application in paper form).

- copies of the passport and taxpayer identification number of one of the parents for the subsequent conclusion of the agreement on the provision of educational services.
